After discussion we draw up a Care Plan, which includes details of what tasks are to be performed, and a copy of this plan is given to the Service User. It is during this visit that our work is explained, including details of whom to contact if you're unhappy with the service and our out-of-hours helpline.

We know that relatives of the Service User often wish to be involved and to have our service explained to them. We therefore welcome relatives be present at this early planning stage and to be fully involved in drawing up the Care Plan. |

Reviews |

All Care Plans are reviewed on a regular basis at intervals which are agreed when the Plan is drawn up. However, we appreciate that things can change suddenly and a review can be arranged at very short notice. |
